(a) Within 24 hours after receiving an initial report described in YTC 12.20.020, the receiving agency shall prepare a written report which shall include, if available:

(1) The name, address, age, and sex of the child that is the subject of the report;

(2) The grade and the school in which the child is currently enrolled;

(3) The name and address of the child’s parents or other person responsible for the child’s care;

(4) The name and address of the alleged offender;

(5) The name and address of the person who made the report to the agency consistent with confidentiality requirements as per YTC 12.20.040;

(6) A brief narrative as to the nature and extent of the child’s injuries, including any previously known or suspected abuse of the child or the child’s siblings and the suspected date of the abuse; and

(7) Any other information the agency or the person who made the report to the agency believes to be important to the investigation and disposition of the alleged abuse.

(b) Receipt of Report.

(1) If Yurok Public Safety or YSS receives a report alleging child maltreatment as described in the definitions section (YTC 12.05.040), the YSS shall immediately initiate an investigation of such allegation and shall take immediate, appropriate steps to secure the safety and wellbeing of the child or children involved.

(2) Yurok Public Safety will assist in the investigation upon the request of YSS.

(3) If a Yurok Public Safety receives a report of known or suspected child abuse or neglect, it shall first attempt to contact YSS in order to refer the case for investigation. If the Yurok Public Safety is unable to contact YSS, it shall make a complete investigation and may institute appropriate legal proceedings on behalf of the subject child or other children under the same care.

(4) The Yurok Public Safety, upon receipt of a report and upon completion of any investigation it may undertake, shall immediately forward a summary of the investigatory data plus all relevant documents to YSS. If YSS is not available for immediate response, the law enforcement agency may take the child into protective custody if there appears to be an immediate threat to the child’s wellbeing pending an investigation by YSS.

(5) Upon completion of the investigation of any report of alleged child maltreatment, YSS shall prepare a final written report on such allegation. Copies of which will be available to the parent or guardian of the subject child upon request. [Ord. 46A § 1301(c), adopted, 3/24/2016.]
